What information do I collect?
I collect only the information required to deliver the service to you. This includes:
Name and contact details
Gender
Pronoun preferences
Occupation
Date of birth
Educational history
Relevant health information (such as your medical history and current situation)
Results of literacy and psychometric tasks involved in delivering the service.
Where do I get your personal information from?
You provide me with the information I collect. This might be when you contact me via email, completion of questionnaires and information gathered during the assessment. Should the assessment have been requested by a third party, for example your employer or education provider, I may also receive and collect information from them.
How long do I keep your information?
Any paper notes I take as part of the assessment will be stored in a lockable filing box and kept securely until the information has been transferred to the report. They will then be confidentially shredded.
Reports and questionnaires are kept in digital form (encrypted PDF) for five years and then destroyed.
What are your rights?
Your right of access - You have the right to ask me for copies of your personal information.
Your right to rectification - You have the right to ask me to correct or delete personal information you think is inaccurate or incomplete.
Your right to erasure - You have the right to ask me to delete your personal information, in certain conditions.
Your right to restriction of processing - You have the right to ask me to limit how I can use your personal information.
Your right to object to processing - You have the right to object to the processing of your personal data.
Your right to data portability - You have the right to ask that I transfer your personal information to another organisation, or to you.
Your right to withdraw consent – When we use consent as our lawful basis you have the right to withdraw your consent at any time.
